Service Authorization & Acknowledgment:
To help ensure a smooth service appointment, please review and acknowledge the following:
Service is billed on a time-and-materials basis unless otherwise agreed in writing.
A minimum labor charge of two (2) hours applies to on-site service unless otherwise specified. Remote and virtual services are billed in 30-minute increments.
Technicians are scheduled within a two-hour arrival window. We will make every effort to arrive within that window and will notify you if delays occur.
Bridge tolls, ferry fees, parking charges, and other applicable travel expenses are not included in the hourly service rate and will be billed separately.
Please have all necessary usernames, passwords, PINs, and access credentials available for any systems, devices, internet services, routers, or software required to complete the requested work.
Please ensure the work area is safe, clear, and readily accessible before the technician arrives. If equipment or furniture must be moved to gain access, we ask that it be moved in advance whenever possible.
FSI technicians will exercise reasonable care while performing services. FSI is not responsible for damage resulting from pre-existing conditions, improperly installed equipment, unstable furniture, or customer-requested movement of equipment or furnishings, except where caused by FSI's negligence.
Customers are responsible for maintaining current backups of important data. While we take reasonable care during service, FSI is not responsible for the loss, corruption, or recovery of data.
Equipment or software issues discovered during service that result from pre-existing hardware failures, software defects, malware, manufacturer defects, or third-party services are not covered unless specifically included in the requested scope of work.
Unless otherwise stated in your service agreement or warranty documentation, invoices are due within fifteen (15) days of the invoice date (Net 15).
By authorizing service, you acknowledge that you have read and agree to these terms. Please refer to your original proposal, service agreement, or warranty documentation for any applicable warranty coverage
